Managing your health can be complex and confusing. Our Care Coordination and Resources team is here to help simplify your care and connect you with resources.

How Our Care Coordination and Resource Services Benefit You

Our dedicated team of navigators, nurses, dieticians, social workers and pharmacists will work together with you to help answer your questions and support you. Our team can help you with your care needs, including:

  • Coordinating your care needs
  • Help navigating the health system
  • Connect you with community resources
  • Provide education
  • Guidance for advanced care planning

Getting Started

Your John Muir Health primary care provider or specialist will make the referral. Then, someone from our team will call you. Most services are provided by phone. If an in-person visit is needed, it will be scheduled at 1914 Tice Valley Road in Walnut Creek.

There is no charge to you for our care coordination and resource services. However, there may be additional charges if you are referred to other services, which we’ll explain before you’re referred for additional care.

You’ll still go to the same primary care provider and specialists you normally see. Our Care Coordination and Resource Services team will work together with your current providers.
